Add Sample Forms Back/Add Form from Template

Early on I figured I’d never need any of the sample forms, so I deleted them. Well, now there is at least one I’d like to use, the Bank Accounts form. Or at the very least I need to create a new form that will be like a Bank Account Registry (just a single account). I can probably figure this out but figure if I can start with a sample, or create my database based on a sample that would be best. I cannot, however, figure out how to get the sample forms back or how to import from a template. When I try to import from a template, it asks me to pick a file. I don’t have a file. Any ideas?
