Between the new iOS and El Capitan, I have no database left. The app crashes on the Mac and there’s no data on my iPhone. Everything is gone even though I backed it up last night.

On the Mac version, can you please make sure that Tap Forms has access to your contacts? Go to Security & Privacy and click on Contacts and make sure that Tap Forms is checked.
On iOS, there is a bug which closes the database just before backing up, but it doesn’t open the database up again after the backup. So what you can do is force-quit Tap Forms and then re-launch it and all your forms will display again.

Oh, and I have fixed that iOS bug and the El Capitan bug for my next update. I submitted the next updates to Apple, but it’ll be a while before they review them.

There is no Contacts in my Security & Privacy section of System Preferences on the Mac. After several hours, I was able to get the DB back on the phone from my backup but I can’t get it to the Mac.
