Hi,
I cannot access my database on Tap Forms 5 on Mac. It turns blank. I checked my Cloudant Sync settings. It is OK. Tap Forms on iPhone and iPad work without problem. I tried re-installing the Mac App but the problem persists. What should I do?

Send the document over from your iOS devices and throw away the bad one on the Mac.
Sorry for the trouble. If you had a power failure on the Mac version this problem could happen.

I was able to send the database from another Mac. But, FYI, Tap Forms crashed when I tried sending from iPhone and iPad. It happened when the iOS app was compressing the database.

How big is your database file? Perhaps it’s quite large.
An alternative way to send the document over is to use iTunes File Sharing instead. It’s also much faster for very large files.
