I did a forum search on this question… In IOS version of Tap Forms am I able to change name of document? From My Life?
I saw this question asked Sept of 2018, but did not see a reply.

I have My Life and now My Life1. I would like to move the forms into one Document.
On the iOS version, close your document, then tap the (i) button at the bottom of the Documents screen. Then tap on the document you want to rename. The Document Info screen will appear and you can rename it there and assign an Emoji icon to it.
To move forms from one document to another, use the Export Tap Forms Archive command via the action menu button on the records list view for your selected form. A .tfarc file will be stored in the Exports folder which you can get to on the Tools tab via the Files screen.
So in Document A, export the .tfarc file. Then open Document B, go to Files, tap Exports, then tap on the .tfarc file and then tap on “Import Archive”. The file will then be imported into the currently opened document.

Brendan, Thanks! I had success with both, moving forms and changing name of document.
I was selecting doc, then hitting “I”… ugh.
