how can I change the Document Icon?

The icon on the encrypted document can’t be changed. But you can change the icon on the other document.
Tap on the (i) button at the bottom of the Documents view. Then tap on the document you want to change the icon for. Then tap on the empty document at the top of the Document Info view.
Then switch to the Emoji keyboard and type in any Emoji character you want. You can use non-Emoji characters too. Tap Forms will just put in a single character above the icon. The house icon you see there is just an Emoji.
